Patient Advocacy: Someone in Your Corner When Healthcare Feels Like a Maze

You have a stack of paperwork, three doctors giving three different answers, and a list of questions nobody has time to answer. If you feel lost, confused, or alone in the healthcare system, you are not alone in that feeling. Millions of patients and family caregivers face the same maze every day.

I’m Peter Abraham, BSN, RN, of Compassion Crossing, LLC, and I work as your patient advocate. I bring years of hospice and palliative care nursing experience to help you understand your care, ask the right questions, and make decisions that fit your life and your values. I work for you, not for an insurance company or a hospital.

What Does a Patient Advocate Do?

A patient advocate is a trained professional who helps you and your family understand, navigate, and manage the healthcare system. Patient advocacy covers many tasks: explaining medical terms in plain language, coordinating between doctors, finding local resources, reviewing medications, and supporting your family through hard decisions.

A patient advocate is not your doctor. I cannot diagnose a medical condition, prescribe medication, deprescribe medication, or change your treatment instructions. Every service I offer is educational and supportive. You and your family will continue to work directly with your doctors and pharmacists on all medical decisions, diagnoses, and prescriptions. My role is to make sure you understand your options and feel prepared for every conversation with your care team.

How I Help: Services Available Anywhere

The following patient advocacy services are available by phone or video, no matter where you live.

Locate Local Resources: Medicine is only part of the picture. I help you find transportation services, meal programs, home health agencies, support groups, and financial assistance programs in your area. I research options and share what I find so you can decide what fits your situation.

Communicate With Your Doctors: When you see several specialists, it is easy for information to get lost between them. I help you organize your medical history, prepare for calls or portal messages with your providers, and follow up on results and referrals so nothing slips through the cracks.

Clarify Medical Instructions: Discharge papers, lab results, and treatment plans are often written in language built for other clinicians, not for patients. I go through your paperwork with you and explain what it means in plain, everyday words, so you know your next steps.

Coaching and Support: Living with a serious or chronic illness is exhausting, and it can feel isolating. I offer ongoing coaching calls to talk through decisions, answer questions as they arise, and help you feel less alone on your health journey.

Keep Family Members Updated: When one person is managing a loved one’s care, other family members often feel left out or unsure how to help. I provide regular updates to family members so everyone stays informed and your loved one gets consistent support.

Research Your Condition and Options: I research your diagnosis, treatment options, and questions to ask your doctor using reliable medical sources. This research helps you have better conversations with your care team. It does not replace your doctor’s evaluation, nor does it guarantee a specific outcome or solution.

Medication Review and Reconciliation: I review every prescription, over-the-counter medication, and supplement you take, checking for interactions, duplicate therapies, and medications that may no longer serve you. I share my findings directly with your doctors and pharmacists, who make the final decisions about any changes. Learn more on the Medication Review page.

In-Person Patient Advocacy in Madison County, KY, and Nearby Areas: Two services require me to be physically present with you, so they are limited to Madison County, Kentucky, and the seven surrounding counties: Clark, Estill, Fayette, Garrard, Jackson, Jessamine, and Rockcastle.

Attend Appointments With You: If your appointment is in Madison County or one of the seven surrounding counties, I can come with you in person. I help you ask questions, take notes, and make sure you understand what the doctor tells you before you leave the room. I do not attend virtual appointments. This service is only available for in-person visits within the service area listed above.

Manage Transitions of Care: Moving between a hospital, a rehab facility, a nursing home, or back to your own home is one of the times when mistakes most often occur. For families living in or moving to Madison County and the surrounding seven counties, I can be present to help coordinate records, ask questions of the receiving team, and make sure important details are not lost during the handoff.

If you live outside this service area, I can still help you prepare thorough question lists and summaries so you get the most out of your appointments and transitions, even though I cannot attend in person.

What a Patient Advocate Cannot Do

It matters to be honest about the limits of this work. I cannot:

Diagnose a medical condition. Prescribe, deprescribe, or change your medication instructions. Guarantee a specific health outcome or a solution to a medical problem. Replace the relationship you have with your doctors and pharmacists.

Everything I provide is educational support, meant to help you understand your health and communicate with your care team. Your doctors remain responsible for your diagnosis, treatment, and prescriptions.

How the Process Works

It starts with a free conversation. I listen to your situation and concerns without pressure or obligation. A plan built around you. Together, you decide which services fit your needs, whether that is medication review, appointment support, or ongoing coaching. Ongoing support. I stay in touch, check in on your progress, and adjust the plan as your needs change.

Fees and Accessibility

Compassion Crossing, LLC, does not accept insurance. Services are private-pay, with a sliding-scale fee structure so support stays within reach across different income levels. If cost feels like a barrier, reach out anyway. That is exactly what the sliding scale is for.
