Hospice Nurse Coaching

Are you uncomfortable answering family questions about how much longer? Do you leave visits wondering whether your charting will hold up under scrutiny?

You chose hospice nursing because you wanted to matter at the end of a person’s life. You wanted to bring calm to chaos, relief to suffering, and clarity to families during their darkest hours.

But amid the documentation needs, eligibility questions, medication reviews, and families seeking answers you’re not sure how to give, the pressure to “do it right” has started to crowd out the reason you came.

You are not alone, and this is not a character flaw. Hospice nursing is among the most demanding clinical fields in healthcare. The knowledge base is wide, the emotional stakes are high, and most of it is expected to be learned on the job.

The Gap Nobody Warned You About

You may have finished your onboarding with a strong clinical foundation and still feel underprepared for the specific demands of end-of-life care. You were not taught how to assess a dying patient’s trajectory over weeks or months. Nobody walked you through how to conduct a meaningful interview with a patient who can still tell you what they need, or how to approach a family when the patient can no longer speak for themselves.

If you are an experienced nurse, the problem looks different but feels just as real. Years of practice can build habits that go unexamined. Documentation patterns that once passed audits start raising flags. Eligibility criteria shift. Recertification language that felt solid last year does not hold up under scrutiny today.

And then there is the part that no textbook covers: sitting with a family at 7:00 PM on a Tuesday when they ask why their mother is sleeping so much, whether she is in pain, or how much longer she has. Knowing what to say and how to say it in those moments is a specific skill. It is not instinct. It is something you build.

Whether you are six months in or six years in, if you have ever left a visit questioning your documentation, your eligibility rationale, or how you handled a family conversation, the gap is real and addressable.

What Actually Fills the Gap

Not a webinar. Not a PDF. Not a forum where advice varies from nurse to nurse.

What fills the gap is focused time with someone who has worked at every level of this care setting and can look at your specific situation, your questions, and your clinical concerns, and give you clear, practical guidance on what to do differently.

That is what Hospice Nurse Coaching with me provides. One hour. One nurse. Guidance you can use on your next visit.

Why Work With Me

I’m Peter Abraham, BSN, RN, EOLD, a hospice nurse and published author of 60 evidence-based clinical guides. I’m a nationally recognized educator who delivered a keynote at the 2026 South Carolina HAPC Virtual Didactics Conference. I’ve managed hospice caseloads with 100% Medicare compliance across multiple organizations and helped one hospice save approximately $55,000 per patient through precise medication management and formulary prioritization.

I’m not teaching theory. I teach what works, drawing on direct patient care, regulatory compliance, clinical writing, and years of navigating the conversations most nurses find hardest to have.

What Others Say About Their Coaching Experience

“As someone new to the world of Advance Care Directives, I was looking for guidance from someone who really knows the ins and outs of working with people during end-of-life, and most importantly, with thoughtfulness and integrity. Boy, did I find that person in Peter! The course I completed was packed with information from someone who is clearly so knowledgeable and kind when it comes to helping people. Not only was the course thorough and eye-opening, but Peter also made himself available to go over questions and created additional content to clarify points that came up during our conversation, helping others with similar questions. It’s refreshing to know Peter is out in the world doing this important work, and being so generous with his knowledge so that those of us who are also passionate about end-of-life care can do the same! I highly recommend working with Peter!!” — Andrea

I have been a hospice nurse for 14 years and look forward to reading more of Peter’s material. This man knows his stuff, comes with receipts, and is very sincere. I would love to collaborate with you in some way in the future, Peter! Thanks so much.” — Steph, Hospice RN, CHNP

Peter Abraham led a continuing education class to our group of Stephen Ministers on the subject of grief. It was full of relevant information on how we can best support our care receivers who are experiencing grief before and after a sick loved one passes away, including how grief shows up among different age groups, including children, and how best to support each age group as they experience grief. It was also very helpful to learn which language is helpful and which is harmful when supporting those who are experiencing loss. Peter was very professional and answered all questions very well. We look forward to having him return to our church very soon. — Edwina

“Peter is an exceptional professional. I attended one of his Dementia seminars. I learned so much about how to better care for my loved ones who are currently in the early stages and was even lucky enough to receive one of his books to assist with caregiving for those with dementia. His seminar covered dietary suggestions, local resources, and even homeopathic suggestions. I can only hope that others can see the value in his work!” — Sara

“We had Peter come and do some extra education for our staff. His compassion and insight have leveled up our caregiving skills.” — Ronda S. Martinez, Assistant Director, Visiting Angels, Richmond, KY

What You Cover in One Hour

Each coaching session is built around your questions. You bring the situation; Peter brings the clinical depth, regulatory knowledge, and practical guidance to help you move forward.

Sessions can cover any combination of the following:

Compliance documentation: Build eligibility-driven, audit-defensible clinical records that accurately reflect what you are seeing at the bedside.

Hospice eligibility: Understand the clinical criteria that support admission and recertification to protect your patients and your organization.

Prognosis and patient interviewing: Learn how to assess a patient’s trajectory and gather the right information from the patient, primary caregiver, and family, including how to approach interviews when a patient’s cognition is limited.

Visit planning and time management: Know what a complete hospice visit entails, how to prioritize your time, and how to structure your day so patients get what they need and you don’t burn out.

Medication review and deprescribing: Develop the clinical reasoning to know when a medication review is needed, how to conduct it, and how to lead honest, compassionate conversations about deprescribing with patients, medical power of attorney holders, and family members.

End-of-life conversations: Build the language and emotional grounding to discuss natural death and dying with families, covering the most common concerns and the approaches that actually help.

What You Take Away

Every session ends with something tangible. Depending on your goals, I will develop tailored handouts to reference what was covered. These can range from a personalized documentation checklist to a structured visit you can follow on your next patient day.

One Hour. $80. Real Progress.

You do not need to take a multi-week course or attend the next scheduled webinar. Book a 60-minute session for $80, pay at the time of booking, and get direct access to an RN who has done this work, written about it, and taught it nationally.

Sessions are booked one at a time. No packages, no commitments. Just the session you need, when you need it.
