Health Navigation: Find Your Way Through What Life Is Throwing at You

If you or someone you love is dealing with a serious illness, remember this: you’re not alone. The healthcare system can be overwhelming, especially when you’re exhausted. It’s common to feel lost amid appointments, paperwork, and unclear decisions, which makes an already difficult situation even more challenging.

I’m Peter M. Abraham, BSN, RN, a Health Navigation Specialist and Patient Advocate at Compassion Crossing, LLC. Based in Berea and serving Madison County and nearby communities, I offer telehealth nationwide. I listen carefully, guide gently, and create personalized plans focused on what matters most to you.

I work independently of insurance companies, so my only priority is you.

What a Health Navigation Specialist Does

I bring nursing experience and knowledge of patient advocacy, hospice, and palliative care to every conversation. I attend appointments in person, prepare question lists, review medications, and help you understand what’s actually being said and what your options really are. My only agenda is yours.

I serve individuals and families in Madison County, KY, and the seven surrounding counties (Clark, Estill, Fayette, Garrard, Jackson, Jessamine, and Rockcastle). For those outside these areas, or when scheduling makes in-person attendance difficult, I can prepare question lists, symptom trackers, checklists, and care summaries to help you make the most of every appointment.

Services for Individuals and Families

Each service below addresses a specific challenge you or your loved one may be facing right now. Click through to learn more about the one that fits your situation best.

Advance Care Planning: Get clear on your wishes before a crisis forces someone else to guess. I help you document your values and healthcare preferences so your voice is heard when it matters most.

Dementia Care Coaching: Caring for someone with dementia is exhausting and often isolating. I support both the person living with dementia and the family members carrying the weight of care.

End-of-Life Support: When someone you love is nearing the end of life, I help you understand what to expect, what questions to ask, and how to make this time as meaningful and comfortable as possible.

Grief Support: Loss changes everything. I offer compassionate support to help you process grief at your own pace, without judgment or a rigid timeline.

Illness Navigation: Facing a serious or chronic illness is hard enough without having to figure out the healthcare system on your own. I help you understand your diagnosis, coordinate your care, and make decisions that align with your values.

Medication Review: If you or a loved one takes multiple medications, the risk of dangerous interactions is real. I review your full medication list and work with your care team to keep you safe.

Wellness Checks: Regular check-ins that go beyond vital signs, focusing on how you are actually doing, what concerns are building, and what support you need to stay well and independent.

Services for Organizations

I also work with organizations that serve individuals and families during life’s most challenging moments.

Staff Development: Training and education for teams working in healthcare, caregiving, and related fields, focused on compassionate, values-based care.

Public Speaking: I’ve spoken to hospice teams, faith communities, and caregiver groups on topics such as navigating a terminal diagnosis, dementia caregiving, and end-of-life conversations. Contact me to discuss your group’s needs.
